A pilot safety-feasibility dietary trial targeting insulin inhibition in ten patients with advanced cancer

چکیده
Background Hyperinsulinemia, Diabetes type 2, and obesity have been identified as increased risk factors for a variety of cancers [1]. Conversely, insulin inhibition (INSINH) can plausibly limit cancer growth by demonstrated mechanisms including ketosis [2] and regulation of downstream signaling proteins such as mTOR (inhibition) and AMPK (amplification), already in development as drug targets [3,4]. Increased F-2-fluoro, 2-deoxyglucose (FDG) uptake on positron emission tomography (PET) scan is characteristic of many aggressive malignancies. We examined safety and feasibility of a four week INSINH diet in patients with advanced PET positive cancers, and compared exit vs. baseline PET scan changes as surrogate measures for tumor response.

Targeting insulin inhibition as a metabolic therapy in advanced cancer: a pilot safety and feasibility dietary trial in 10 patients.
OBJECTIVE Most aggressive cancers demonstrate a positive positron emission tomographic (PET) result using ¹⁸F-2-fluoro-2-deoxyglucose (FDG), reflecting a glycolytic phenotype. Inhibiting insulin secretion provides a method, consistent with published mechanisms, for limiting cancer growth. METHODS Eligible patients with advanced incurable cancers had a positive PET result, an Eastern Cooperati...
متن کاملAloe vera Leaf Gel in Treatment of Advanced Type 2 Diabetes Mellitus Needing Insulin Therapy: A Randomized Double-Blind Placebo-Controlled Clinical Trial
Background: Advanced type 2 diabetes mellitus (T2DM) needing insulin therapy is a common disease. Previous studies indicate that aloe (Aloe vera L.) leaf gel may positively affect the blood glucose and lipid levels in patients with advanced T2DM needing insulin. Objective: Evaluation of the efficacy and safety of aloe leaf gel in the treatment of type 2 diabetic patients resistant to ...
متن کاملFeasibility of preoperative chemotherapy for locally advanced, operable colon cancer: the pilot phase of a randomised controlled trial
BACKGROUND Preoperative (neoadjuvant) chemotherapy and radiotherapy are more effective than similar postoperative treatment for oesophageal, gastric, and rectal cancers, perhaps because of more effective micrometastasis eradication and reduced risk of incomplete excision and tumour cell shedding during surgery. The FOxTROT trial aims to investigate the feasibility, safety, and efficacy of preop...
متن کاملThe Individualized Diet and Exercise Adherence Pilot Trial (IDEA-P) in prostate cancer patients undergoing androgen deprivation therapy: study protocol for a randomized controlled trial
BACKGROUND Androgen deprivation therapy (ADT) is the foundation of treatment for men with metastatic prostate cancer and is now frequently incorporated into multimodality strategies for the curative treatment of locally advanced prostate cancer. Nevertheless, the catabolic effects of ADT result in meaningful adverse effects on physiological and quality of life outcomes, which may, in turn, incr...
متن کاملThe potential of honey to promote wound healing in periodontology: a pilot randomized clinical trial
Abstract Background & Objectives: Honey has been used as a nutraceutical in many traditional and ancient remedies. Considering well documented benefits of honey to accelerate wound healing, for the first time we aimed to assess intra oral surgical wound healing process with honey. Methods: We designed a pilot randomized placebo controlled cross-over clinical trial. Patients who required bilate...
